The aim of the current research is to identify the effectiveness of a program in medical mathematics based on cognitive integration in the time of the Covid-19 pandemic to develop health numeracy and mathematical identity among middle school students. To achieve this goal, the researcher used the quasi-experimental method with two groups: experimental and control; where the experimental group and control group consisted of (54) male and female students, in the first semester of the academic year (2021-2022). The research relied on two tools: health numeracy test and mathematical identity scale. independent -samples t test, one-sample t test, and ETA square were used in the statistical data processing. The results of the research indicated the effectiveness of the proposed program in developing the health numeracy, and the mathematical identity among the experimental research sample. It is recommended to the need to pay attention to the cognitive integration between the fields of mathematics and health and the practices of teaching mathematics related to the emotional aspects, especially the mathematical identity in the mathematics teacher preparation programs through courses of teaching methods and practicum, and holding training courses for mathematics teachers, and their mentors in how to employ the concepts of health numeracy in the implementation of mathematics curricula and how to enhance the mathematical identity of their students, as well as the inclusion of enrichment activities in the mathematics curricula that enhance the students health numeracy in the different stages of education.
